A proof of concept pilot was set up in Waitemata for heart failure patients who remained under the care of General Medicine. The pilot ran from August 2022 - until August 2023. There were two goals of the pilot; 1) evaluate the acceptability and feasibility of a new remote patient monitoring model of care, 2) evaluate the usability of a supporting digital platform.
