Organisation: Northland District Health Board

The Planned Care Integration Programme aims to address three key areas:

1. Improving access to Planned Care Enable patient-centred booking by way of the development of an API for the patient booking system and telehealth solutions and improve whānau input into care planning with enhancements to Whānau Tahi Shared Care Platform

2. Transformation of patient pathways Implementing change to improve patient journey and experience for planned care

3. Improving the primary/secondary care interface Implementation of new pathways, referral processes and access to planned care provision in the community via collaboration with NGOs.
